Human decision on every recommendation
Handlers accept or override each assessment. Overrides require a reason category and a written explanation, and high-risk overrides are held for a second reviewer's approval.

Assessments, decisions, approvals and status changes are logged with actor and timestamp, filterable by claim and event type, and exportable to CSV.
